Ziggy came with a "Canine Health Record" - basically a piece of paper that gave his whelp date, labels from the vacs he was given and a six-digit "puppy number".

Can anyone tell me if this indicates that he was a puppy mill puppy, or does it maybe sound like the the rescue filled out the form...?

There is no identification on the paper itself one way or another.

The reason I am averse to doing that, is that the "rescue" was a home in the heart of Detroit with some very iffy characters in it.

Your dog could very well have come from a mill... but they wouldn't issue a #. My dog came with some dog registration number from the store, a second registration (which was not AKC... although many "mill dogs" are given AKC#s).... also a registry# (for microchip) since the original owners did follow up with that...

The # was probably issued by the rescue... which could be legit... or adopting out "pet store failures"... ie: weren't sold fast enough and outgrew their cuteness.

What is the name of the rescue?
