Has anyone ever seen this before my dog shakes his head alot like if his ear hurts or itches I will attach a foto of what his ears look like. Please help!!!!!!

Oof, that looks nasty! He definitely needs to see a vet so you can figure out what's going on. I'd be worried that it's inflamed like this inside the ear too, which can definitely cause temporary or permanent deafness if it's not addressed. It's against forum rules to give any medical advice for something that hasn't been diagnosed by a vet, or to guess at diagnoses, so we won't be able to help you anywhere near as much as your vet can!
