I'm working on the intake form / behavior history for Tyson's behaviorist appointment later this month. It's hard! And long!

Any advice from anyone who's done this before?

Is it a standardized form/ the c barq?
I think some of the sections are based on c-barq, but there are other sections for descriptions of the reasons and goals for the appointment, details about his issues and methods used to address his issues, general training methods and known cues, and daily life. It's about 20 pages. Mostly I don't want to sound stupid.

I did one for Watson. Overall I didn't find it that hard. Most of the stuff was n/a to his situation (aggression issues, etc). Mostly I had to be careful to really put in the relevant information so it was obvious why he was there because there was a lot of irrelevant stuff.

What are you finding challenging? I'm not sure if the form is the same thing we filled out though I assume it's similar.
